Thank you for scheduling an appointment at Positive Eye Ons Optometry with Dr. Stybel. We look forward to your visit! You can learn more about our office here on our website. For your convenience, our patient welcome and history forms can be found by clicking this link.

We encourage you to complete your forms online and submit them to us electronically. Alternately, you can choose to complete the forms when you arrive at the office and we ask you to arrive 10 minutes early to do this and bring with you a list of your current medications.

When you arrive for your appointment please bring all of your insurance card(s), including any vision and medical insurance cards. Please bring all prescription eyewear you may have, including sunglasses, computer glasses, or glasses used for hobbies/sports.

Our office is located at 940 Fairfax Ave West Hollywood, CA 90046, across from Starbucks. Parking is available at meters on Melrose, and there also is 2 hour Street parking on Stanley north of the alley.
